Filling your Prescription

If you need a refill on your prescription, please call during office hours, 8:00 am to 5:00 pm, Monday through Friday, at 970-382-9500.

To facilitate efficient handling, please request refills during regular office hours by leaving a message which includes:

  1. Your name
  2. The type of medicine(s) you need refilled
  3. The pharmacy you use

The message will be forwarded to your physician or the physician on-call, and your prescription refill will be called in to the pharmacy as soon as possible.

Please check with your pharmacy to see if your prescription has been approved before arriving at the pharmacy. Certain prescriptions are available by written refill only and will only be filled during regular business hours. The office staff will inform you if your pharmacy needs the prescription to be written.

If you have questions and were unable to locate the answer within this website, or if you wish to speak with us personally, please call us at 970-382-9500.

Our office staff can assist you in obtaining your medical records, refilling prescriptions and/or with any problems or questions.

If you wish to speak with a physician, the staff will take your name, telephone number and a brief description of your question. A physician or a physician assistant will return your call most likely that same day.

Your Medical Records

On April 14, 2003, the privacy standards of the Health Insurance Portability and Accountability Act (HIPAA) were implemented throughout the medical industry.

We want you to know that we take your privacy seriously. We are committed to doing our best to safeguard the accuracy and security of your health information, and we will follow all federal, state and local regulations while doing so. Because of the many new rules in place, there may be times when we ask you to fill out paperwork that you haven’t had to fill out in the past. We apologize in advance for any inconvenience this may cause you, and thank you for your patience and understanding as we work together to keep your information safe.

Please refer to our Privacy Notice, which will provide the details of when we can and cannot release your information, according to HIPAA.

When requesting copies of your medical records/X-rays, we ask that you please give us at least 48 hours notice to prepare your request for pick-up. We will complete disability and attending physician forms for you if they are duly authorized; however, you may be charged a fee for the completion of these forms.

For your convenience, you may mail the records/X-ray release request to us or drop them off at the receptionist’s desk, with specific instructions as to how you would like them sent out for completion. Be advised that if our office staff does not recognize you, we may ask you to provide identification before releasing any records.
